Information We Collect
This site collects information in two ways: through the contact form on our Contact Us page, and through standard web server log data (such as IP address and browser type, retained by our hosting provider). If you accept our cookie banner, we also collect Google Analytics usage data as described below.
Contact Form
Our Contact Us page has a form for Name, Email, Phone, and Message. When you submit it, that information is sent by email — through our transactional email provider, Resend — directly to our office inbox. We don't store form submissions in a separate database; they exist only as an email in our inbox, subject to our normal email retention practices.
Please don't use this form to share sensitive health information. It's meant for general inquiries and scheduling requests, not a secure channel for protected health information. Google Analytics
With your consent, we use Google Analytics to understand how visitors use this site — for example, which pages are viewed and how long visitors stay. Google Analytics uses cookies to collect this information, which may include your IP address, browser and device type, and pages visited. This data is processed by Google under the Google Privacy Policy. You can opt out using the Google Analytics Opt-out Browser Add-on, or by blocking third-party cookies in your browser settings.
Google Analytics only runs if you accept our cookie banner. Declining, or leaving the banner unanswered, means it never loads.
Google Maps
Our Contact Us page can embed a Google Map to help you find our office. Like Google Analytics, this only loads after you accept the cookie banner; if you decline, we show a plain link out to Google Maps instead of the embed.
Cookie Choices
Your choice is remembered in your browser's local storage, not on our servers. You can change your mind at any time by clearing your browser's site data for this domain, which will show the cookie banner again on your next visit.
Children's Privacy
Our website is not directed to individuals under the age of 13, and we do not knowingly collect personal information from children.

We may update this policy from time to time. We'll update the "Effective" date above when we do.
